To submit a form electronically, 1. Click the link to the form, 2. Fill out the form, 3. Click the down-facing arrow near the top right-hand corner of your screen, 3. Select "Without your changes" if you would like to fill it out in a PDF application or select "With your changes" if you would like to fill it out on the current webpage, 4. Save as a PDF after you have filled out the form, 5. Email it to the applicable email listed on the form.
